What is Assist – Life Stage, Transition?
Assist – Life Stage, Transition (also known as Support Coordination for transitions) is an NDIS Capacity Building support that helps participants navigate significant changes in their lives. These transitions can include moving from school to employment or further education, being discharged from hospital, entering or leaving residential aged care, and changes in family situation or living arrangements.
At Disability Dream Care, our transition support specialists work proactively with participants, their families, and other service providers to ensure that every major life change is planned carefully, communicated clearly, and managed with the least disruption possible. We help identify what supports are needed before, during, and after the transition — and we stay alongside you until you feel settled and confident in your new situation.
We recognise that transitions can be emotionally and practically challenging. Our team approaches each transition with deep empathy, strong organisational skills, and thorough knowledge of the NDIS system — ensuring participants don't fall through the gaps at the moments when consistent support matters most.

Who Is This For?
This support is ideal for participants who are approaching a significant life change and need structured guidance and coordination to navigate it successfully. It is particularly valuable for young adults finishing school, participants being discharged from hospital or rehabilitation, those moving from family homes to independent or supported living, and older participants transitioning into aged care. Families and carers also benefit from this support as it reduces complexity and ensures nothing important is overlooked during the transition period.
