Community Nursing Care

Qualified nursing care delivered in your home and community — clinical expertise with a personal touch.

What is Community Nursing Care?

Community Nursing Care is an NDIS support delivered by registered and enrolled nurses in participants' homes and community settings. It provides access to professional clinical care outside of hospital environments, enabling participants with complex health and disability support needs to live safely in their own homes without requiring unnecessary hospitalisation or residential care.

At Disability Dream Care, our community nursing team brings together highly experienced nurses who specialise in supporting people with disability. They work collaboratively with GPs, specialists, allied health professionals, and support workers to provide coordinated, person-centred clinical care. All nursing staff hold current AHPRA registration and relevant clinical competencies required for the tasks they perform.

Nursing care can be provided on a regular scheduled basis, on an as-needed basis for specific clinical tasks, or in response to changes in health status. Our team maintains detailed clinical documentation and communicates proactively with the participant's broader health care team to ensure continuity of care and early identification of any emerging concerns.
